Level I Services

thumbnail image The Level 1 Waiver Services are for individuals with developmental disabilities who meet the waiver’s eligibility requirements.

Please schedule an assessment with Ford Care Services, Inc. Call 614-317-3099 or set an appointment online to know if your family member or loved one qualifies for this service.

Through Level I Services, the individual may be allowed to stay at home and get support rather than be confined in an Intermediate Care Facility (ICF/MR).
